Payments

Patients are responsible for payment of their procedure. Before your day of surgery, we will verify eligibility with your insurance carrier. We will work with your physician to obtain prior authorization if required. We will call you to confidentially discuss your benefits and give you an estimate of your financial responsibility that you will be required to pay prior to surgery. A written estimate may be requested, and all patients will receive a written estimate the day of your surgery. Cash, VISA, Mastercard, Discover Card and American Express will be accepted. Your insurance company will determine your final charges based on the service you actually receive. After we receive payment from your insurance carrier we will mail you a statement for any remaining balance as determined by your insurance explanation of benefits. Uninsured

For patients with no insurance we offer cash discount if payment is made on the day of surgery. This discount does not apply for cosmetic surgery services as defined by our facility. If a patient’s services are not found to be covered by insurance or third party payment source, the uninsured discount may be disallowed. We will call you to confidentially discuss your financial responsibility that you will be required to pay prior to surgery. Collection Procedures

If needed, the center will attempt to reach a patient by any method available to us to secure payment on any outstanding balance utilizing internal and external resources. If the account becomes delinquent, it may be placed with an attorney or agency for collection in which their fees and expenses may be the obligation of the patient.

You will be billed separately by your physician, anesthesia provider, any pathology, radiology or laboratory used and our third party implant payer. Please contact the following providers to see if they participate in your health insurance plan.
